Overview: Join our team as a Housing Coordinator and play a vital role in empowering individuals with disabilities to secure stable and affordable housing. You'll engage in meaningful interactions with clients, offering personalized support to navigate the complexities of housing. Your responsibilities will include conducting comprehensive assessments, devising tailored strategies to overcome obstacles, and connecting clients with valuable community resources. As a pivotal member of our team, you'll manage a diverse caseload, adjusting your workload to accommodate varying needs and challenges. Embracing a person-centered approach, you'll conduct home visits and hold meetings at various community touchpoints, ensuring accessibility and convenience for those we serve.
In this position, you will:
- Regularly meet with clients to discuss housing options, progress, and troubleshoot issues.
- Thoroughly document client interactions in Pavillio.
- Ensure that a minimum number of billable hours are maintained each week.
- Provide transportation for housing-related matters.
- Guide clients through the housing search, application, and screening processes.
- Aid clients in applying for public programs to cover housing costs and accessing subsidized housing waitlists.
- Identify resources for deposit payments, moving expenses, and assistance, securing aid when possible.
- Facilitate relationships with prospective landlords and promote cultural understanding.
- Assist clients in planning and executing their moves.
- Support clients in maintaining their housing and housing benefits and preventing returns to homelessness.
- Advocate for clients facing eviction or housing instability.
- Perform community outreach and liaise with other local agencies.
- Help clients develop natural community supports to boost housing stability and retention.
- Perform other duties as assigned.
Required Qualifications:
- Be at least 18 years or older with a bachelor's degree in social work, psychology, sociology, human Services, or related field OR minimum two years of related experience.
- Have experience working well with a diverse population, including individuals who have low income; struggle with substance abuse; have criminal backgrounds; are involved with Child Protective Services; and people with developmental, mental, and physical disabilities.
- Have proof of valid driver’s license, proof of insurance, and personal vehicle.
- Be willing to travel within the metro area and transport clients as necessary.
- Be able to pass a Department of Human Services background check.
- Be a team player who is dependable with a strong work ethic.
- Possess the ability to manage time effectively.
- Have strong communication and interpersonal skills.
- Demonstrate professionalism in the workplace and in the community.
- Have a commitment to empowering individuals to take accountability for their lives and realize their fullest potential on their own terms.
- Be able to navigate basic technology, such as electronic charting systems, Microsoft Word, etc.
